Why did the City Office ask whether I lived long term in Japan before?

I came to register my new address. I had lived in Japan as a student 4 years ago, but didn't know I had to pay national health insurance, which I didn't for 2 years and left the country. I'd received bills from the Health Insurance every month back then but ignored them. Now I'm back with a new residence card to update my address and they're asking if I'd lived in Japan long term before this.

What are the chances of having to pay all my late health insurance costs altogether?
